MEDICAL, DENTAL AND VISION CARE

PATIENT ELIGIBILITY

To establish care at Mission Health Lexington call a member of our team at 859-272-0219. At that point you will do an initial phone screening to determine eligibility. If determined eligible for services you’re required to submit proof of the following via email or in-person. Once the documents are received you’ll be eligible to schedule an appointment. 

  • 18 years of age or older (drivers license, passport, etc.)

  • Live permanently in Lexington, Kentucky (utility bill, lease, etc.)

  • Be uninsured (we are able to verify this)

  • Meet our income requirements (3 months of bank statements for the household, prior year taxes, etc.)

INCOME REQUIREMENTS

Mission Health Lexington serves individuals whose household include is at or below 185% of the Federal Poverty Level. Current annual income amounts for 2025 by household size are as follows:

  • 1 person: $28,952
  • 2 people: $39,127
  • 3 people: $49,302
  • 4 people: $59,477
  • 5 people: $69,652
  • 6 people: $79,829
  • 7 people: $90,002
  • 8 people: $100,177
