FOR REFERRING PROVIDERS

Patients must be referred to Faith Pharmacy AND deemed eligible for services before they are able to fill prescriptions. Patients must live in Lexington, be uninsured, and have a household income at or below 185% of the federal poverty level. Please refer to our formulary before writing/sending prescriptions as we have a limited inventory of medications available. Our referral form is below. After we receive the referral form and list of medications we will contact the patient to determine eligibility, collect required documentation, etc. Please do not send prescriptions until we verify eligibility. With the pharmacy only open on Saturdays it is very difficult to transfer prescriptions. If you have any questions prior to referring a patient please email or call us and we are happy to guide you through the referral process.

HOW TO FILL A PRESCRIPTION ON SATURDAY

Faith Pharmacy is open on Saturdays only. You must be a registered Faith Pharmacy patient in order to fill a prescription. Once we receive a referral from your provider, our pharmacy team can determine your eligibility for pharmacy services and the availability of your prescribed medications. If you are eligible for pharmacy services you must arrive between 8:30am – 10:30am to request an initial prescription or refill.

HOW TO BECOME A PATIENT

To become a patient at Faith Pharmacy a representative from your physician’s office must complete a referral form and send a list of medications. We will contact the patient to determine eligibility, collect required documentation, etc. Please do not have prescriptions sent or transferred until we verify your eligibility. With the pharmacy only open on Saturdays it is very difficult to transfer prescriptions out. You must be a Lexington resident and meet our income requirements. If determined eligible you’re required to submit proof of the following via email or in-person prior to filling a prescription. 

  • Photo ID (drivers license, passport, etc.)

  • Proof of Lexington address (utility bill, bank statement, etc.)

  • Meet our income requirements (3 months of bank statements for the household, prior year taxes, etc.) Mission Health Lexington serves individuals whose household income is at or below 185% of the Federal Poverty Level. Current annual income amounts for 2025 by household size are as follows:
    • 1 person: $28,952
    • 2 people: $39,127
    • 3 people: $49,302
    • 4 people: $59,477
    • 5 people: $69,652
    • 6 people: $79,829
    • 7 people: $90,002
    • 8 people: $100,177
